Responsibilities and Duties:

Serves as the primary point of contact for telephone patients across the life span with a variety of health or illness questions

Utilizing a patient centered approach, collects subjective data related to the presenting problem and medical history; accesses patient information via EMR

Utilizes critical thinking and problem solving skills to recognize and match symptoms to protocol, assigning acuity (emergent, urgent, acute and non-acute) and ensuring safe, timely disposition of health related calls

Offers home treatment advice as appropriate

Documents encounter and decision making in EMR in a concise and accurate manner, utilizing the patient's own words as appropriate

Utilizes principles of active listening to elicit patient information; communicates based on patient’s level of understanding and comprehension

Maintains dialogue and navigates computer programs to assess, process, educate and document information contemporaneously

Demonstrates knowledge and use of the nursing process to identify physical, psychosocial, and developmental needs and interventions to promote favorable outcomes

Participates in staff meetings, in-services, committees, and continuing education opportunities; adheres to organizational and departmental policies, procedures, and established best practices

Always observes and maintains confidentiality of protected health information
Other duties as assigned

Education/Experience/Certification/Licensure:

Accredited Nursing Program graduate; BSN

preferred

Active nursing license for USA, multistate license under the Enhanced Nurse Licensure Compact (eNLC) preferred; specific state licensure may be required to obtain prior to commencing role (reimbursed)

Minimum of 3 years of clinical nursing experience – ER/Urgent Care, Ambulatory Care, Pediatric, OB/GYN or Home Health preferred

Experience with EPIC EMR is preferred

Physical Requirements:

Ability to sit for long periods of time within a confined space

Requires full range of motion, manual and finger dexterity of eye hand coordination

Requires corrected vision and hearing to a normal range

Sedentary work involves sitting most of the time but may involve walking or standing for brief
