Applications in Triplicate on PSC form 3 (revised 2008), with three recent colored passport photographs and bearing the Title applied for and reference number should be addressed and submitted to the:

Applicants should attach certified copies of all the necessary supporting documents, academic Transcripts, registration and valid practicing licenses (where applicable), National IDs and any other relevant document.
Serving Officers who wish to apply should route their applications through their Heads of departments.
