    Responsibilities

  • Receives incoming calls, parcels and visitors and channel them appropriately
  • Provides requested information and takes message with courtesy.
  • Keeps record of documents in a retrievable manner according to AHF guidelines
  • Files both incoming and outgoing communications in the right places
  • Creates and maintains filing system in an organized manner.
  • Types letters, memos and ensures they are delivered to the relevant places/people at the right time.
  • Analyzes documents for approval and dispatch them once approved.
  • Ensure adequate stocks of office supplies, inventory and office utilities.
  • Track assets, inventory and office equipment such as computers, printers, phones and furniture.
  • Resolves work related conflicts and builds team work among staff at the facilities.
  • Manages and supervises all maintenance workers at the host center.

  • Ensures proper patient record keeping on ART, Pre ART and other records according to the M.O.H guidelines and stock keeping on all drugs at the CCC.

    Responsibilities

  • Conduct preliminary interviews/counseling for ART to selected clients so in order to assess client’s immediate needs, including medical, housing, mental health, transportation and other supportive services;
  • Measure client’s adherence level to routine medications such as septrin and ART and assess barriers to and supports for adherence;
  • Periodically reassess adherence level, barriers and supports, and make a record of all findings as well as plans generated during counseling;
  • Conduct community outreach to locate clients who are out of care or need support to remain in care;
  • Identify resources and promote program; serve as liaison to community by conducting presentations on all program activities.
  • Participate in community activities such as SEEP, health fairs, special events;
  • Assist with case management duties including accompanying client to referral appointments and case conferencing with service providers to ensure optimal service delivery to clients;
  • Track and record in ART and other relevant registers and program databases all activities undertaken;
  • Ensure the maintenance of appropriate, accurate, and clear documentation of all outreach, education, case management and group activities – report writing;
  • Assist program development and clinicians to explore for and identify additional linkage to Care services such as legal help, nutritional support; Identify resources and promote program;
  • serve as liaison to community by conducting presentations on all program activities;
  • Actively participate in trainings, on-going skills enhancement trainings, and network meetings;
  • Establish a clear bi-directional process with medical care providers for the receipt and provision of relevant information on the medical and psychosocial status of enrolled clients;
