Avenue Healthcare Chronic Care Lead Nurse Jobs in Kenya; Perform care management for chronically ill patients with conditions such as chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and congestive heart failure. Work in collaboration with patients, family/caregivers, clinic providers, and community resources to increase patient self‑management and shared decision‑making.


Key Responsibilities

  • Fulfill Avenue Healthcare’s mission to provide high‑quality care that exceeds client expectations.

  • Manage registry of chronic care management (CCM) patients and validate enrollment based on provider requests.

  • Comply with documentation requirements by recording care plans in EMR with patients, families, and providers.

  • Monitor adherence to care plans, evaluate effectiveness, and facilitate changes as needed.

  • Create ongoing processes for patients and caregivers to determine desired level of care coordination.

  • Facilitate patient access to appropriate medical and specialty providers.

  • Coordinate transition of inpatient to outpatient care to reduce readmission rates.

  • Collaborate with providers to manage symptom control, medication management, and patient/family education.

  • Educate patients and families about relevant community resources.

  • Identify and enroll high‑risk patients into the registry.

  • Coordinate continuity of care with external healthcare organizations and facilities.

  • Support patient self‑management
    and behavior modification interventions.

  • Provide health counseling, education, and instruction.

  • Perform any other duties assigned by the immediate supervisor in line with the job description.


    Person Specification

  • Diploma or Degree in Nursing from an accredited institution.

  • Valid license with the Nursing Council of Kenya.

  • Updated BLS and ACLS/ATLS certificates.

  • Minimum 3 years’ experience in healthcare involving patients with complex chronic disease states.

  • Customer focus, empathy, and emotional intelligence.

  • Strong interpersonal and communication skills.

  • Organizational awareness and attention to detail.

  • Team‑oriented and results‑driven.

  • Ethical and reliable.
