    Responsibilities:

    Major Duties and Responsibilities:

  • Provide emotional support and initial adherence counseling for patients who are newly enrolled to care and treatment program
  • Provide a follow up adherence counseling for those exhibiting inadequate compliance to the program and those identified to require additional support
  • Identify patients who missed their appointment, LTFU and Dropped from Follow up and do tracing as per the national strategy using standard tools
  • Do home visit to client who aren’t able to reach the clinic & those who need additional support
  • Facilitate the organization of adult support group and monitor their regular meeting
  • Facilitate the referral and linkage of patients who need the support of other related organizations (i.e. CBO, FBO, governmental organizations)
  • Monitor the distribution of basic preventive care package kits and other supportive materials
  • Support the clinical team (ART nurse, ART Data Clerk) in daily registration and patient chart extraction and arrangement.
  • Ensure the documentation of all job specific activities: patient referral, tracing registration, adherence counseling registration, kit distribution registration, and appointment calendar
  • Submit monthly/ quarterly/ annual activity report as per the agreed reporting template
  • Execute adherence support related duties as irected by ART
  • Nurse-in-charge, Clinical Officer, ART Clinician or Medical Director

    Qualifications:

    Education, Qualification, and Experience

  • Minimum of secondary school completion or certificate in any field of study
  • At least two years direct experience in adherence counseling, ART treatment support, and related HIV client support
  • Experience and training as an Expert Patient, ART adherence support, or other training on HIV patient care
  • Experience and training on Home Based Care or Palliative care provision is an advantage
  • Enjoys working as a member of a cohesive multi-cultural team
  • Good communication skills in Amharic is required (English language speaking is an advantage)

    Responsibilities:

    Major Duties and Responsibilities:

  • Provision of ART Treatment support and follow up counseling for clients who may have adherence related challenges and complience to care
  • Provide emotional support and initial adherence counseling for patients who are newly enrolled to care and o the treatment program
  • Identify cases and families who may require additional support and thus develop care plan as per the standard recommendations
  • Using the appointment calendar / TB DOTs as standard tool, identify patients who missed their appointment, LTFU and Dropped from Follow up and do tracing as per the national strategy (home to home, telephone, using the treatment support group).
  • Ensure to have
    appropriate and updated contact address for all enrolled
  • Do home visit to client who aren’t able to reach the clinic & those who need additional support
  • Assist in the facilitation of adult support group program, together with thte Adherence Supporter, and monitor their regular meeting
  • Ensure the documentation of all job specific activities: patient referral, tracing registration, adherence counseling registration, kit distribution registration, and appointment calendar
  • Execute adherence support related duties as directed by ART Nurse-in-charge, Clinical Officer, ART Clinician or Medical Director

    Qualifications:

    Responsibilities:

  • Essential Duties & Responsibilities:
    In close coordination with the facility staff, ensure the provision of Comprehensive, quality, evidence based ART (including early initiation for the eligible and ready, PMTCT, TB/HIV co- management, OI management, and Prevention with positive (Pwp), STI/SCM), Care for HEI, management of malnutrition, FP integration, retention to care, PEP, Emergency cases and treatment failure
  • Takes the lead role in coordinating and provision of HIV management for Clinic’s Patients with more emphasis to pediatrics cases and taking care of complicated cases
  • Participate in the management of adult clients as deemed necessary
  • Coordinate and Monitor PEP service being a focal person at the clinic
  • Facilitate the referral and linkage of patients to community service and to service outlet not available in the clinic

    Qualifications:

  • Education, Qualification, and Experience
    University graduate in Medical Doctorate from a recognized university
  • At least three years direct experience in patient management of ART, PMTCT, and TB/HIV in a similar institution
  • Experience in clinical program management; Must be highly organized and capable of effective multi-tasking
  • Training on ART management would be a necessity; additional training on PMTCT, HCT and TB/HIV, MDR
  • TB, Nutritional management for PLWH is an advantage
  • Experience with operational research and evidence based healthcare is an added advantage
  • Enjoys working as a member of a cohesive multi-cultural team, and is good at doing so
  • Basic computer/software skills and abilities required
  • Good communication skills in English and Amharic
